*Appointments booked online are subject to approval and are based on physician’s orders, insurance and/or authorization, medical necessity.

We are open Monday through Friday from 10:00 AM to 6:00 PM (closed for lunch from 1:30-2:00 PM).

*Our current policy is a $50.00 charge for cancellations that are not given 24 hours in advance.
